Using Docket®

District residents can access personal and/or family immunization records on file with the District of Columbia Immunization Information System (DOCIIS) through the Docket app.

Use of Docket® is 100% optional. It is available for free as an app for both Apple and Android devices. You may also access it on a computer web browser. Docket® is intended only to provide consumers with access to their personal and family immunization records and offer reminders for recommended immunizations. Docket® does not sell or disclose your personal data to any third-party apps, services, or websites.

Setting-Up Docket®

Step 1: Download the “Docket® - Immunization Records” app from the Apple App Store or Google Play Store or navigate to https://docket.care on the web.

Step 2: Sign-up or log in to Docket® using your email address, Apple ID, or Gmail account.

Step 3: Search for your immunization record through the Docket® app:

  • Your name, date of birth, and legal sex entered in Docket® must match your District of Columbia Immunization System (DOCIIS) record exactly.
  • Phone number or email address verification is required to access a digital copy of your personal and/or family immunization record.

Record Management

Adding Family Members
Tap the plus (+) icon on the Immunization Search History screen to add family records to a shared Docket® account. Children, 17 years old and younger, can be associated with their parent/guardian.

Downloading DC Health Patient Vaccine Record
Select the PDF icon to access a PDF copy of your official District of Columbia immunization record, which you can download from Docket®.

Receiving Immunization Reminders
Enable notifications to receive push notification reminders for upcoming and overdue recommended immunizations according to public health guidelines. Select the calendar icons to import all-day reminders to your Apple, Google, and/or Outlook calendar apps.

Frequently Asked Questions (FAQ)

Q: Why can’t I access my records using the Docket® app?

A: Your record may not be accessible in Docket® for several reasons.

  • Your record may not be stored in District of Columbia Immunization System (DOCIIS). Many providers did not routinely report vaccines to DOCIIS 2.0 until 2021. Additionally, your provider may not have shared your immunization record data with DOCIIS.
  • If your record is in DOCIIS, and you still cannot access it, the contact information you entered in Docket® may not match your DOCIIS record. If you need assistance locating your records or need to update your contact information, please email [email protected] along with a photocopy of your current state issued ID or picture ID.

Q: I can’t verify my identity using Docket® because my contact information that appears in the app after I search my records is incorrect. How do I correct my email address and/or phone number on file?

A: Contact information updates to your DOCIIS record can be sent via email to [email protected] along with a photocopy of your current state issued ID or picture ID.

Q: How do I refresh my Docket® account with new immunization data?

A: Drag to refresh your Docket® mobile app just like you would refresh your email inbox or social media feed. If you’re using the Docket® app, select the recycle button to refresh your account. Your new immunization data should normally appear after a few seconds as long as it was correctly reported by your doctor or pharmacist to DOCIIS.

Q: My Docket® account is still missing my latest immunization data even after I refresh. What should I do?

A: Ask your doctor or pharmacist to report your latest immunization(s) to DOCIIS. Make sure that your provider has your updated phone number and email address in their system. Then, refresh your Docket® account after you have received confirmation that your latest dose(s) has been reported to DOCIIS. It may take some time (ex. one week) before your doctor submits your vaccine information to DOCIIS and it is reflected in Docket®.

Q: Is Docket® the only option to request my District of Columbia immunization record?

A: You may also obtain your immunization records over email by completing the “District of Columbia Immunization Information System (DOCIIS) Release of Information Form.

Email the completed form to [email protected] along with a photocopy of your current state issued ID or picture ID. DC Health will send back your full immunization record in an encrypted email within 48 business hours after you send the signed Release of Information Form.

Q: Is Docket® a vaccine passport?

A: No. Docket® is not a vaccine passport. Docket® is offered as a service for you to access your personal and/or family (e.g., a dependent) immunization records contained in DOCIIS.

Q: Can I access my complete immunization history?

A: Docket® displays all your immunizations that are entered into DOCIIS by your healthcare providers. There are various reasons why certain immunizations may not have been recorded in DOCIIS, notably, if they were administered outside of DC; please check with your healthcare provider if your DOCIIS record is not up to date.

Q: What data are you storing?

A: DOCIIS is a secure statewide immunization registry. Data contained in DOCIIS includes limited demographic information (e.g., name, contact information), details about what immunization had been received and when, and whether any immunization series are currently due or past due.
